Frasco Profiles is committed to working with victims of human trafficking to block certain adverse items of information from appearing in their consumer report.

How to Request a Block of Information in Your Consumer Report

The Fair Credit Reporting Act (FCRA) requires the following information from any person requesting a block of information on their background check. When contacting Frasco Profiles regarding an adverse information block, please provide your preferred method of communication for us to contact you about your request.

You will need to provide the following:

1. Proof of Identity

  • Proof of identity can be established in one of two ways:
    • Consumer File Match – Provide your full name (first, middle, last, suffix), any previously used names, full social security number and/or date of birth.
    • Additional Proof of Identity – Copies of government issued identification documents, utility bills, and/or other methods of your identity, which may include answering questions to which you may be expected to know the answer.

If you have a designated representative submitting information on your behalf, then in addition to your proof of identity, your designated representative needs to submit their proof of identity and a valid power of attorney or some other documentation that authorizes the representative to act on your behalf.

2. Adverse Items of Information You Wish to Block

  • Provide the specific item(s) on your report you are seeking to block.

3. Trafficking Documentation

You are required to provide an acceptable Victim Determination statement, which is a document that contains a determination of human trafficking, made by either:

  • A Federal, State, or Tribal governmental entity.
  • A non-governmental member of a human trafficking task force, including victim service providers affiliated with these entities and authorized by a Federal, State or Tribal government entity.
  • A self-attestation by a consumer who identifies as a victim of human trafficking that is signed or certified by a Federal, State or Tribal government entity, court of competent jurisdiction, or an authorized representative of these entities.
